Our Service

The Australian Vietnamese Aged Care Services is a Vietnamese ethnic specific home, and the facility provides 24/7 nursing care and access to Vietnamese speaking staff. Being of the same heritage and cultural background as the residents, the staff have formed a strong bond with the residents and treat them as their own family members. The care team provides person centered care and always strives to provide safe, quality, effective and efficient care to the residents. The home has access to a 24-hour Emergency call system.

The Facility provides 24/7 with qualified and skilled Registered Nurses to lead the care team and to provide specialized care and treatment to the residents. The Registered Nurses and the care team are led by an experienced Care Manager/Clinical Supervisor and a Clinical Nurse Educator. The clinical governance framework is supported by evidence practice such as Infection Control Management, Medication Management, Wound Management and Palliation Management. We have successfully transitioned to the New Accreditation Standards supported by Policies and Procedures and Processes. Individualized and personalized care plans are developed in consultation and collaboration with the residents and representatives, with choice, dignity and respect underpinning this process.

Personal care services are provided adopting the person-centered care approach such as personal hygiene with showering preference time to preference with dining experience.

The Catering Service provides meals and diets that are in accordance with Food Safety Program and meet the nutritional requirement designed by the Dietitian. Meals are culturally respectful of the Vietnamese heritage and developed to meet the needs of the residents.

The Facility provides onsite Allied Health Services such as Physiotherapy, Podiatry, Dietetics, Speech Pathologist and Audiologist.

Lifestyle and Recreational Activities play an important in the well being of the residents. A comprehensive monthly activity calendar provides activities that are meaningful, interactive and respectful of choice for the residents. AVACS celebrates festivity occasions, Birthdays and cultural events. External entertainers regularly perform to the residents at AVACS, not to mention a team of highly talented care team who perform for the residents during celebratory occasions. The home is decorated with Vietnamese heritage arts and memorabilia to allow the residents to be able to reminisce the past. Residents religious and spiritual needs are considered with leisure and lifestyle care planning. Christian and Buddhist groups attend regularly at AVACS to facilitate the spiritual and religious needs of the residents.

A team of dedicated Vietnamese and non-Vietnamese speaking General Practitioners provide care to the residents. Medical care is also supported by the local Geriatric Palliation Outreach team and the Fairfield Geriatric Flying Squad who review the residents at AVACS to provide Palliation or end of life care.

Pharmacy Service provides medication advice and supplies for the residents, with delivery of medication and pharmaceutical supplies twice a day.

Local pathology services come on site to collect blood and mobile X-Rays and available on request.

The Board of Directors works closely with AVACS on the Organizational Governance, to promote cultural safety, diversity and respect, dignity of risk and choice.

 

Overall the home design allows for responsive and flexible care in line with each individual’s changing care needs in a familiar and appropriate environment. The home aims to provide residents with a non-institutional and more personalised care environment based on a person-centered care model.
